• Name, grade, and age of child
• Reason for transfer
• New address, if transfer is due to change of residence.
• Name and address of the school the child will attend
• Last date the child will attend St. Joseph's.

After parents sign the necessary release, our school office will forward the appropriate information and records, provided that all obligatinos to the school/parish have been met.
